Forms - Enrolment Information

Enrolment in public schools is a two stage process. The first phase involves lodging an Application for Enrolment form with the school. This form will generally be assessed within 24 hours and you will then be notified regarding the success of your application. If you are successful you will then be required to complete formal enrolment details. Sometimes, more time is needed to ensure that special needs of some students can be managed and planned for. In these instances, the school will liaise with parents or carers to facilitate formal enrolment.

When you lodge your Application for Enrolment with the School Officer please ensure you have attached a copy of the following documents:

  • Birth Certificate 
  • Proof of Address (recent utility bill)
  • Immunisation History Statement (available from the myGov website or Medicare)
  • Visa Grant Notice (if applicable)
  • Student Health Care Summary 
  • 2018 Consent Form
